Photo/Video Consent Release Form

Backflow Solutions, Inc. (BSI Online), Chicago Backflow Inc. (CBI), and Red Star Inspections, Inc. (RSI) are committed to professionally promoting their businesses and services. As part of our promotional, advertising, marketing, and training efforts, we may use photographs, videos, other visual recordings, and brief written summaries of you and the associated work history of our employees and/or provided by employees for internal and external purposes, including but not limited to:

• Social media posts
• Website content
• Marketing materials (flyers, brochures, etc.)
• Advertising campaigns (printed and electronic)
• Training resources
• Company summaries for public procurement documents

Consent: By signing this release form, you grant BSI, CBI, and/or RSI the right to use, reproduce, alter, and publish photographs, videos, or audio recordings in which you may appear and/or be heard. This consent applies to both past, current and future materials, and you waive any right to royalties or compensation for their use.

Scope of Use:

1. The media may be edited, altered, or used in part or whole without inspection or additional approval.
2. The materials may be used across various platforms and mediums, including print, digital, and online.
3. The consent applies worldwide and does not expire unless otherwise revoked in writing by the employee.

Revocation of Consent: You may revoke this consent by providing written notice to BSI/CBI and/or RSI by emailing marketing@backflow.com. Upon receipt of your revocation, the companies will make reasonable efforts to discontinue future use of your image or recordings. However, previously published materials cannot be recalled.

Acknowledgment: I understand and agree to the above terms. I confirm that I am providing this consent voluntarily and without duress.

A PDF will be sent to the email entered below for your records.

Background:

• Please make certain you are standing in front of a plain, one colored background. This is as simple as a white or light-colored wall in your home or office.

Portrait or Live Focus Mode:

• Please make sure your phone is set to “Portrait Mode or Live Focus” in your camera app.
• On iPhone – ensure Live Photo is OFF.
• This will focus solely on the subject, while slightly blurring the background to best capture the image.
• This gives your headshot the professional look we are aiming for.
• Please provide landscape and vertical options.

Attire/Poses:

• BSI, CBI, or RSI shirt preferred.
• If no company shirt – an appropriate plain shirt with no patterns or other logos.
• No pajamas.
• Be ready for the photo!! (makeup, hairstyle or clothes you want to wear – we will not edit).
• Smile J (if you do not want to smile, that is ok too, just a friendly look on the face will do!). This is not a mugshot photo, but rather a friendly face to promote our businesses.

Filters:

• No filters or editing yourself before sending to us.
• Please do not send us a photo from Snapchat or one used on social media.

Editing:

• We will not be spending our time editing or photoshopping items outside of our design control: teeth coloring, hair color, makeup, glasses, or any other personal detail.

We are simply applying to a company background and lighting/brightening, if the lighting is off.
